I found out what the issue was for me. For some reason, melee kills with a hammer didn’t count. When I switched to a knife, the kills started tracking again.

True Warrior is stuck at 64% for me. Going to the all achievements tab did not fix the issue for me unfortunately. Guess I'll have to start a new game. If that doesn't work then I don't know what else to try.
Yeah it’s stuck at 20% for me as well. Considering it’s unlock percentage is quite low it could be that it’s bugged for everyone?
No specific advice but in general, delete your profile from console and re-downloading it sometimes helps with sticky achievements. I got an achievement I thought was unobtainable pop from doing that before, and it could 'cleanse' your tracking.
Other options are to use DIFFERENT melee weapons (Mantis Blades work a treat), and to kill different types of enemies.
CDPR have been very prompt thus far in patches, so I would be shocked if this wasn't addressed very soon.
I noticed when I was grinding out melee kills for this one, that if the enemy dies due to any extra weapon effects (Bleeding, shock, etc.) that the progress doesn't go up. Seemed to work better if I charged a heavy attack and killed the enemy in one shot. Although seeing as so many people are stuck, there is clearly an issue with this achievement.

I’ve tried deleting my profile, resetting the cache on the Xbox, unplugging it, redownloading my profile. I’ve tried it with mantis blades, katanas and knives.
I’ve tried - Reloading the mall save with tonnes of enemies and killing all of them and waiting for a save or just going to my main game and doing some the light blue missions (so they weren’t save scummed enemies) so they were enemies I’ve met for the first time in Free roam missions. Might try gorilla fists tomorrow.
I’ve got 20 enemies towards it but it’s not progressed in days.
The Witcher 3 has now ruined all games for me! Can see why it won all of those Games of the Decade awards...

Hit the Xbox button on your controller, go to achievements and you'll see "all achievements", and usually that's enough to force update any of your progress. I always go to the game's list too just to make sure.

I've been doing that since 2011. It's never updated a stuck chievo for me. Not sure those mechanics work. Think that was more coincidence that you experienced. My True soldier is stuck at 96% and The Quick and the Dead at 92%. So far all my others are still tracking and unlocking as necessary. Glad this game came on my One X Cyberpunk console. I just returned the collector's edition.

I have 4 the achievements for completing river and kerrys story, collecting all of johnnys belongings as well must be rats thats stuck at 86%

Just before finishing playing tonight I started doing some of the area missions for Santa Domingo - four to be exact but the tracker for achievement ‘The Jungle’ hasn’t moved off 0%. Anyone else got this?

the ones for completing the areas 100% arent completable all at once. Sometimes you have to progress in the story and youll get a new gig or ncpd event. Some dont count towards the achievement at all, or count towards another borough's achievement.
Also, make sure your game is up to date because sometimes it doesnt boot you off until a few hours after the update comes out. Thats what it did and i didnt unlock the judy vs night city achievement, i had to reload an autosave before the trigger after updating.
No achievement glitched on me, it just may take you a while longer to get something because you havent done everything yet.

Nope. Not the case. I'm 60 hours in, 5 main story lines completed. This is an RPG. I'm off exploring, no such thing as progressing an achievement tracker for killing based on story position.

I’ve done every single thing I possibly can. missions, gigs, side job etc and my Little Tokyo achievement is stuck at 96%.
Literally the only thing I have left as an option to do is the final story mission. Ugh.

^Just want to update in case anyone else is stuck and out of options like I was. deleting my local save and syncing back to the cloud save got my last achievement to unlock. Was stuck at 96%.
Can’t wait for more content. What great time playing this was.

Yeah no, that doesn't work either. I even played on my wife's Series X and they still do NOT unlock. The tracking and therefore unlocking is pure randomness.
The trackers for each district are still working on mine, and one unlocked as it should. So, the different methods we are all using to unlock these have no rhyme or reason. Other than this title should've been delayed another year.

Other areas have stuck or tracked poorly for me but the achievements eventually unlocked.
Now I’m stuck on the get wood area - I’ve completed everything the achievement guide says I need but the tracker shows me at 90% and no achievement.
Also with the gig ‘On a tight leash’ I completed it but it shows still in my journal. If I try to track it nothing happens and weirder still ive just been back there and the place is empty apart from the target - he was there and I’ve killed him again but nothing happened. Really confused now

Solved it - loser back in this evening and as I loaded the auto save right next to having killed him again it told me I had failed the gig - 3% was added. Then I did the gig at the top right of the map which is just into city centre but actually counts for heywood and finally one of the cyberpsychos - the one in the car garage - counts for the last couple of percent. Job done in the end!

Frequent flyer has been stuck at 96% for me. I found an additional 4 or 5 data terms but it never increases. I also cross referenced locations with a list from powerpyx. Lots of people saying this popped early for them, but I’ve gotten every single one confirmed and still at 96%.

Keep hoping they fix this in the NEXT update.
I deleted this from my Series X. As well as the save file. Reinstalled it and still no joy. Wife has same achievement stuck at 46%. So I thought hey, I'll install this on my Taco Bell Xbox One X. Well.....NOPE, still stuck. So it's not a console specific problem. It's just sh*t assed programming. This game culture is starting to get irritating what with us, the goddamn gamers being the qa/ qc testers, and NOT GETTING PAID FOR IT!!
